Applying for care

Applications for midwifery care are submitted through our secure intake form, which is hosted by Ocean (CognisantMD). Information you enter there is handled under Ocean’s own privacy practices.

Contact form

When you send us a message through our contact form, we keep your name, phone number, email address and message so that our office can reply. Please do not include personal health information in the form. Messages are only visible to our office staff.

Cookies

This website does not use advertising or tracking cookies. A single cookie is used only when our staff sign in to update the website. Our fonts are loaded from Google Fonts.

Maps and address search

The catchment area map uses OpenStreetMap. When you check an address, the address you type is sent to the OpenStreetMap Nominatim service to find its location. We do not store it.
